Please remember, if you are admitted to Hollywood Hospital and would like to see one of our volunteer hospital visitors, please tick the box marked, TPI and HOSPITAL VISITORS. This will ensure that one of our volunteers will visit you. Should you have any needs or complaints, our hospital visitors will do their utmost to attend to your concerns.
Please note: Each time you enter the hospital as a patient, you will need to complete a Patients Personal Details Form.
IF YOU ENTER ANY OTHER HOSPITAL AND WOULD LIKE A HOSPITAL VISITOR, PLEASE INFORM THE TPI ASSOCIATION.
